Everybody wants to produce extraordinary outcomes. No one, no leader, no organization, strives to be mediocre or ordinary. Young athletes dream of becoming Pelé, Mickey Mantle, Michael Jordan. People want to be great and be part of a team or organization that is creating something great.

And yet, it is not so easy to translate what seems to be a universal desire into actual performance and results. Why is that? What gets in the way? Why do leaders and teams who are smart, motivated and committed often not achieve their highest aspirations for the future? What does it take for a leader to create the kind of organization in which these aspirations are realized?
